Remote Care Coordination Specialist Healthcare

As a Remote Care Coordination Specialist, you will serve as a vital link between Aetna members, healthcare providers, and our internal clinical teams. Your primary goal will be to ensure that members navigating complex medical situations — from hospital discharges to chronic disease management — receive timely, coordinated support that keeps them on track with their care plans. Your Day-to-Day

  • Coordinate post-discharge care plans for members transitioning from hospital to home or post-acute settings
  • Liaise between Aetna's clinical teams, primary care physicians, specialists, and community resources
  • Contact members by phone and digital channels to assess care needs, confirm appointments, and address barriers to care
  • Process referrals and prior authorization requests in accordance with clinical guidelines
  • Maintain accurate and up-to-date member records in Aetna's care coordination platform
  • Monitor member progress against care plan milestones and escalate concerns to clinical managers
  • Support members in accessing community health programs, social services, and durable medical equipment

Who You Are

  • You have 2+ years of experience in care coordination, case management support, or healthcare administration
  • You have strong knowledge of managed care concepts, referral processes, and prior authorization workflows
  • You communicate clearly and compassionately with members from diverse health literacy backgrounds
  • You are organized, detail-oriented, and capable of managing a large caseload efficiently
  • You are familiar with HIPAA standards and healthcare privacy regulations
  • You hold at minimum a high school diploma; a medical assistant certificate or healthcare-related associate's degree is a plus
